An
Insured Person who desires to
avail medical benefit on
superannuation from insurable
employment shall submit an
application to the Branch Manager
in the prescribed form along with
a declaration from the employer
concerned in the prescribed form.
The Branch Manager after examining
the application alongwith the
other documents and with reference
to the records maintained in the
Branch Office shall, after
satisfying himself about the title
of the insured person, record his
orders in the prescribed form. Once
the Branch Manager certifies the
eligibility of an IP to the
medical benefit under the scheme,
the Insured Person is required to
deposit a nominal contribution of
Rs. 10/- per month in advance for
one full year (Rs. 120/-) with the
Branch Office. Once the annual
contribution is deposited in
lump-sum the applicant shall be
issued a fresh Photo
Identity card for himself and his
spouse that will remain valid for
one year. A fresh Medical Record
Envelope shall also be
prepared and forwarded to
the ESI Dispensary opted for
the by the insured person.
Admissibility to Medical Benefit
and the photo identity card can be
revalidated at the Branch office
by depositing one year's
contribution in advance.
