Medical
Benefit to retired
& disabled persons
Eligibility
condition

|
For
retired persons
An
Insured Person who leaves the
insurable employment on
attaining the age of superannuation
after being insured continuously
for not less than 5 years,
immediately before superannuation,
becomes eligible to Medical
Benefit for himself and his/her
spouse at the scale prescribed
under the ESI Act and the
regulations made thereunder.
Further Insured person has
to produce the proof of his
superannuation and having been in
insurable employment for a minimum
period of 5 years immediately
before superannuation to the
satisfaction of the officer
authorised by the Corporation for
the purpose.

For
disabled persons
The
disabled insured person willing to
avail medical facility is required
to produce the proof to the effect
that he has ceased to be in
insurable employment on account of
permanent physical disablement due
to an employment injury to the
entire satisfaction of the officer
who has been authorised by the
Corportion to accept the
case. Along wiht an
application in a proper format,
the disabled peson shall also
submit a declaration of permanent
disablement in the prescribed
format.

Contribution
The
superannuated or disabled insured
person whose eligibility to
medical care has been
authenticated by an authorised
officer of the Corporation will
make payment the normal
contribution @ Rs. 10/- per month
in lumpsum for one full year in
advance at the concerned branch
office of the Corporation that
will entitle the insured person
and his/her spouse to medical
care under the Scheme.
