What is Acrobat
Reader, and why do I need it? Adobe® Acrobat® Reader™ is
free software that lets you view and print Adobe Portable
Document Format (PDF) files.
PDF files cannot be
altered, and the PDF format ensures that documents are printed in an exact
fashion, with correct page-breaks and margins. It is for these reasons
that we chose to provide our Social Security forms in PDF
format.
How do I get the
free Acrobat Reader?
If you do not already have this
software, You can download Adobe® Acrobat® Reader™ by clicking the icon
below, then filling out some basic information about your computer. This
file will take between 30-60 minutes to download.
How do I download
the forms? Once you have downloaded and installed the Acrobat Reader, just
click on the ESIC Forms
that you require. The file will download and open automatically
in your computer.
What do I do once
the form has been downloaded? Print your form after it has been
downloaded. Fill out the form and take it in to your nearest ESIC Branch. It's that easy.
